Chronic Pain and Opioid Stewardship - North
St. Joseph's Care Group brings 20 years of experience in the delivery of an interprofessional chronic pain self-management program. This very experienced interprofessional team, consisting of Occupational Therapy, Physiotherapy, Kinesiology, Psychology, Recreation Therapy, Social Work, Dietitian, and a consulting physician brings clinical perspectives and a breadth of care essential to meeting the needs of this complex population.
The Ottawa Hospital brings experience with outreach models such as Pain Connect -- a pain management preceptorship that takes place in the office of a family physician with patients. Specific advice on pain assessments, including addiction screening, as well as management options tailored to the practice and the needs of the patients is offered. The team also uses eConsult, a secure electronic communication program that allows primary care physicians to post questions on particular patients and receive a response back within 7 days.
Together, SJCG and TOH bring many years of experience working closely with rural physicians and have intimate knowledge of the challenges they face. In Northwestern and Northeastern Ontario, access to specialty clinics or even single providers with expertise in the management of chronic pain is far more limited than in more populated areas of the province. The geography is vast and treatment centre’s are sparsely distributed. Travel for treatment is challenging especially during the winter months. Expenses related to travel, to access assessments, care or services can be beyond the means of many people. Access to training for primary care providers is more difficult in the North. For both these reasons, ECHO offers a way to build capacity, knowledge, and confidence in primary care and experience in rural and remote communities in Northern Ontario. ECHO offers an opportunity for networking and a way to share case presentations in a safe and non-judgmental environment.
~Moving knowledge, not people. The result: Better care for more people, closer to home.~
